Issue:
A Veteran found medical papers on three Veterans in a conference room and gave to a nurse. Contains three Veteran's information, for two Veterans, full SSN and medication orders and for one Veteran full SSN, DOB, Medication Orders, EKG and progress notes. Items were printed on 8/29/12 but unsure if left at that point or more recent. One Veteran has since past away. Update: 11/06/12:Two (2) Veterans will be sent letters offering credit protection services, and one Veteran's next of kin will be sent a notification letter.
Outcome:
Letters mailed. Unable to determine what staff left papers.
