Issue:
Two VA0750 Performance Evaluations forms were added to the OPM folder on employees ending 09/30/11. Both of the employee performance evaluations were scanned incorrectly into the wrong file. The information released incorrectly was full name, Social Security Number, and Performance Evaluation information involving two employee files. Discovery was brought to the attention of Human Resources on 02/28/12 by one of the employees reviewing his OPF file. Update: 03/01/12:Two (2) VA employees will receive a letter offering credit protection services.
Outcome:
Employee staff were reminded on the importance of safeguarding patient personal identification.
