Issue:

A VA Administrative office was broken into at Rock Valley Community Program Grant Per Diem Program. The safe was broken into, the police were notified, and the premises searched. It is unknown if Veteran files were reviewed. The hard copy files are still intact. Update: 04/05/12:The community based transitional living facility administrative office was broken into including the file cabinet containing Veteran patient/resident information including medication lists, SSN, CPRS progress notes for 31 Veterans. The files appear intact but were visible. File cabinet contained Veteran records, safe and bus passes. The focus of the break in appears to be monetary the sheriff's office is still investigating. Since Veteran information was exposed, thirty-one (31) Veterans will receive a letter offering credit protection services.

Outcome:

Staff at the facility had secured the records and have transferred them to another secure area file cabinet following the break in. No reason at this time to assume the records were looked at or anything removed although the possibility exists. Electronic record keeping was suggested to the program as a possible alternative to current paper copy procedures. Office is being re-secured. Window was unlatched - possibly from staff and may have been means of egress but entry was apparently through locked office door.
