A Release of Information (ROI) clerk inadvertently mismailed a Veteran's medical records. The law firm that received the records in error returned the records to the VA. Privacy will investigate.
Outcome:
04/14/15: The Incident Resolution Service Team has determined that the Veteran will be sent a letter offering credit protection services due to full SSN being disclosed.
