Issue:
A patient went to the emergency department (ED) and received the wrong individuals discharge instructions. The instructions included: name, SSN, and DoB as well as information from the visit. The patient didn't notice he had the wrong information until he got home. He went into the Community Based Outpatient Clinic (CBOC) the following day and provided the document. Update: 01/15/13:Patient B will be sent a letter offering credit protection services.NOTE: There were a total of # Mis-Handling incidents this reporting period. Because of repetition, the other # are not included in this report, but are included in the "Mis-Handling Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Staff have revised processes to assure that another individual reviews before the information is provided to the Veteran. Training is ongoing. .
