Issue:
On 6/23/2012, Veteran A received an appointment letter reminding him of an upcoming appointment. The Veteran was surprised to see a second appointment letter, which was intended for another Veteran. Veteran A contacted the Privacy Officer by phone to report the incident. The medicine service line chief is being notified and Veteran B appointment letter will be mailed to the correct address. A formal letter requesting that Veteran A return Veteran B appointment letter to the Dorn VAMC is being mailed. The information disclosed is full name, partial social security number, address, and the name of the provider. Update: 06/25/12:Veteran #2 will be sent a notification letter.
Outcome:
STAFF HAS BEEN RE-EDUCATED ON THE NEED TO SAFEGUARD PATIENT INFORMATION.
