Issue:
A Veteran was referred by Nashville VA to a transitional housing facility (per diem grant program) in Lewisburg, Tennessee.On Friday, 09/09/11, the Veteran obtained a list of his medications through the Release of Information (ROI) department, Nashville campus, which he gave to the case manager, transitional housing facility. The Veteran stated the case manager told him the facility was required to keep a list of the Veterans medications.Later in the day, the Veteran found the medication list, under the TV set, in the living room of the transitional housing facility. The Veteran stated he purposely left the documents in place through the weekend to monitor if the information would be retrieved.Monday, 09/12/11, the Veteran took pictures of the documents, retrieved the information, which is now in his possession, and reported the incident to the Privacy Officer, Nashville campus. The Veterans Personally Identifiable Information (PII) includes: Full name, full SSN, and date of birth. Update: 09/12/11:The Veteran will be sent a letter offering credit protection services.
Outcome:
The case manager did not follow the admission process. The case manager was counseled regarding the importance of safeguarding PII. In the future, PII will be kept in a locked filing cabinet in locked office. CM letter mailed.
