Issue:

The Environmental Management Service (EMS) cleaning staff found a 1 inch Compensation and Pension (C&P) file sitting in the lobby. There was no one around it. The EMS staff turned it into the eligibility staff who contacted the Privacy Officer (PO) and C&P staff. C&P staff involved in the clinic and involved in the patients care are not sure how it got there. Update: 09/26/12:One Veteran will be sent letters offering credit protection services due to full name and full SSN being exposed.

Outcome:

All staff in the area were questioned and importance of patient privacy and safeguarding sensitive information was presented.. The last person to remember having it was the C&P physician who thought he put it back on the file in the C&P Office.
