Report ID: SPE000000069743, U.S. Department of Veterans Affairs

Reported Entity: VISN 01 Bedford, MA

Issue:

A VA employee at the Lynn Community Based Outpatient Clinic (CBOC) handed the Veteran her medication bottle and a printout of her medication reconcilliation. The Veteran did not understand how the employee was involved with her care and why she was allowed to have access to her medical information. Update: 12/16/11:The employee was identified as a trained volunteer peer specialist with a documented background check. Voluntary Service does have some conflicting/missing documentation. The volunteer was told that paperwork had to be in order before she could go back to work. The volunteer is scheduled to meet with the Voluntary Service Chief on 12/19/11.12/29/11:The volunteer's status was terminated due to her not reporting for work during her hours. Since she was technically not a volunteer at the time of the incident, this is being re-classified this as a privacy violation. The Veteran will receive a letter of norification.

Outcome:

Volunteer met with Chief of Voluntary and completed all necessary training and orientation documentation to return her status to an active volunteer.
