Issue:
A VA employee delivered education materials to an affiliate employee. Included in the materials was a letter to a patient. The letter was a reminder to schedule an appointment. The letter included the patient full name, and address on it. The affiliate employee reported the disclosure to the VA immediately and returned the letter. Update: 05/08/12:The patient will be sent a notification letter due to PHI being exposed to someone who would not have had access to the information during the course of his/her duties.
Outcome:
Ordered a separate printer so patient information will not get mixed in with educational material.
