Issue:

Today, 12/13/13, Patient A and Patient B were being discharged from 7B (Murfreesboro Campus). During the process, discharge medications for both Patient A and Patient B were given to Patient A who was being transported to his home in Memphis, TN. Patient A was contacted, travel notified, and the transport re-routed to the Memphis VAMC where Patient A is to return Patient Bs discharge medications to the Pharmacy. Memphis VAMC Pharmacy has been alerted to the incident and is expecting the medications to be returned and destroyed. Patient B and Patient Bs next of kin were notified prior to discharge from the facility.

Outcome:

12/16/13: Patient B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
