Patient A was provided with Patient B discharge papers. Patient B's full SSN, first name, demographics, and medical information was exposed. Update: 02/10/12:Veteran B will receive a letter offering credit protection services.02/24/12:After further review, Patient A had it for less than a minute and turned it right back in. The risk of a data breach is extremely low, but the site chose to send a HIPAA notification just to make sure Patient B was notified.
Outcome:
Staff was retrained.
