Issue:
Veteran A, while meeting with the Facility Director, and while discussing a variety of issues, also brought up the issue of him receiving a copy of a Hospital Inquiry (HINQ) belonging to Veteran B. Veteran A provided the document to the Director. The protected health information (PHI) located on that HINQ inquiry included full name, full SSN, DOB, address, service time and branch of service.
Outcome:
09/16/14: The Incident Resolution Service Team has determined that Veteran B will be sent a letter offering credit protection services.
