Issue:

Today, a Lead Medical Support Assistant, Nursing Service, Nashville Campus, found a patient label on the second floor stairwell outside the Richard Franc Conference Room, Nashville Campus. The patient label belonged to a Veteran who was recently hospitalized on 2N and contained the following: full name, last four digits of the Social Security Number, and ward location. The label appears to be the type used by the residents during Grand Rounds. There are no security cameras in the area but the stairwell is fairly isolated and primarily used by physicians. The Veteran, to whom the information belonged, was hospitalized for four days. The label was found two days after his discharge.

Outcome:

05/27/15: The Incident Resolution Service Team has determined that while there was the potential for unauthorized access/disclosure of data, it has been determined that the incident has a low probability of a risk of compromise.
