Issue:

A VA Pharmacy employee removed two sheets of VA Form 10-2320 (Schedule II and Schedule III Narcotics and Alcohols) from the pharmacy vault. The information contained the drug name, date, prescription number, patient's last name, first initial of the first name, quantity dispensed and balance of stock. The employee admitted to taking this information home. He left work at 4:30 PM 07/28/11 and returned the information at 8:00 AM 07/29/11. Update: 08/08/11:The list contained 44 Veteran's information. It was taken home by the employee so that he could respond to disciplinary action. Since the sheets were out of control and contained Last names, first initials, and medication information, HIPAA notification letters will be sent to all 44 Veterans.

Outcome:

Management is pursuing appropriate disciplinary action. Privacy Training and Rules of Behavior current per Service Chief. Internal investigation ongoing. It appears that there were 44 Veteran names (last name and first initial of the first name) on these two sheets.
