Issue:
The Privacy Officer (PO) received a call today from Veteran A. Veteran A received medical records in the mail belonging to Veteran B. Veteran A stated that there was approximately 80 pages total. The top page is a cardiac report of some sort. Veteran A disclosed the name of Veteran B whose records he received. Veteran B's name, address, date of birth, full SSN and protected health information were compromised.
Outcome:
04/30/14: The Incident Resolution Team determined that Veteran B will receive a letter offering credit protection services.
