Issue:

The Chief of Health Administration Service (HAS) contacted the Privacy Officer (PO) about Veteran A receiving Veteran B's (who is also an employee of this facility) medical information. Veteran A was sent to the Chief of HAS because Release of Information (ROI) is part of his section. Chief of HAS contacted the PO to report the incident and turn over the documents. The Chief of HAS has made contact with the ROI supervisor who will get back with the PO after talking with employee and reviewing if Veteran A who received wrong documents was awaiting his own medical information. Update: 08/01/12:Veteran B will be sent a letter offering credit protection services.

Outcome:

Supervisor of Release of Information (ROI) instructed staff to check and double check information being mailed out. Employees have been instructed to check address on every piece of paper with the one showing on the envelope to be mailed.
