Issue:

In early June 2014, a Veteran received VA medical documents via United States postal channels; however, the documents of another veteran were also enclosed in the same mailing. The veteran returned the documents to the facility where they were mailed from and did not want name disclosed for their return. The veteran did not attempt to contact other veteran, nor did the veteran attempt to copy or otherwise use PHI/PII data of the other veteran. A review of the record sent in error indicates it is complete. The document of the other veteran has been returned from where it was mailed and a letter of notification will be sent to the veteran whose data was received in error. Upon mailing of notification letter recommend this this ticket be closed.

Outcome:

06/26/14: The Incident Resolution Service Team has determined that Veteran A will be sent a letter offering credit protection services.
