Based on record review, the facility failed to protect the private health information (PHI) of Patient A. This failure resulted in a breach of Patient A's PHI.The facility Breach Report, dated 5/16/13, was reviewed on 5/16/13. The report indicated PBX operator (PBX O) released a copy of Patient A's death certificate to the wrong mortuary. The document was returned to the facility. The death certificate contained the following information: Patient A's name, address, telephone number, date of birth, race, date expired, time of death, nearest relative's name, address and phone number. Employee PBX O was re-educated and counseled.
