Privacy Officer received a telephone call from Veteran A. Veteran A reported that his Provider was unable to print a document during a recent visit for Veteran A to sign, so the Provider stated he would mail the document to Veteran A. Upon receiving an envelope from the VA, Veteran A opened and found a Benzo Agreement to sign, however, he then noticed the document was not his and it contained the full name and full SSN of Veteran B. Veteran A has the document and will keep it secure until he can return it to the Privacy Officer which will be next week.

Outcome:

09/25/13: Veteran B will be sent a letter offering credit protection services.
