A Veteran employee requested copy of his Sensitive Patient Access Report (SPAR). He identified names on the list that should not have accessed his chart. The Privacy Officer (PO) conducted a fact finding investigation. The chart was inappropriately accessed. Update: 07/20/12:The Veteran employee will receive a letter offering credit protection services.
Outcome:
Verbal counseling has been provided to staff.
