Findings:The facility was made aware of a breach of patient medical records information on January 23, 2014. The facility notified the incident to the Department on January 23, 2014. A staff from Medical Records mailed a patient's medical information on January 23, 2014, to the wrong address while attempting to mail to patient's therapist. The letter was received by a private business in error that contained patient's name, date of birth, date of admission, doctor's name, age, sex, medication regimen and discharge progress notes. The error was discovered when the business contacted the facilty to notify about it. Administrative staff confirmed the incident during a telephone interview on January 27, 2014 at 9.00 am.
