Issue:
On 01/27/15, Veteran A called this VA to report that he picked up his Outpatient medications on 01/23/15 from the Outpatient Pharmacy and when he got home, he then noticed that in his medication bag was a bottle of medication meant for Veteran B. Veteran B's full name, home address, telephone number and the type of medication was on the medication bottle. Pharmacy has already contacted Veteran A for the return of the medication.
Outcome:
01/28/15: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
