Issue:

An electrocardiogram SD memory card was lost. The card possibly contained information on 30 veterans, with SSN (24 of them had the full SSN, 5 had the last 4 of the SSN, and 1 did not have the SSN) along with electrocardiogram data and date/time of the test. A search resulted in not locating the card.

Outcome:

06/13/14: The Incident Resolution Service Team has determined that 24 Veterans will be sent letters offering credit protection services and six will be sent notification letters.
