Issue:
One Pharmacy Tech mailed a prescription to the wrong Veteran and 1 PSA clerk changed an address which led Pharmacy to mail another presc ription to the wrong Veteran. The Veterans' names, addresses and medication information were disclosed. The date of birth and SSN were not disclosed. Update: 09/21/11:Veteran A and B will be sent a notification letter.
Outcome:
Staff have been educated regarding changing addresses and mailing out prescriptions.
