Issue:

A Veteran received a CD and paper records which contained the personally identifiable information (PII) and/or protected health information (PHI) of another Veteran. The records were mailed to Veteran A's home. The records were for Veteran B. Veteran A returned the records to the facility Patient Advocate. Update: 08/20/12:Veteran B will be sent a letter offering credit protection services. His full SSN and medical information were mailed to Veteran in error.08/27/12:Upon further investigation, 2 Veterans' SSNs were disclosed. Both will be sent offers for credit protection services.NOTE: There were a total of 22 Mis-Mailed incidents this reporting period. Because of repetition, the other 21 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.

Outcome:

CM letters mailed. PO provided education to employee. HR and service has completed appropriate disciplinary action.
