Issue:
Patient A came in for a nurse appointment on 04/12/11 and gave the nurse a medication list that he received in the mail. The medication list was for Patient B and was printed on 04/05/11. It is believed that the patient lists were mismatched to the letters when they were mailed. The medication list included Patient B's name, full SSN and medications. Update: 04/19/11:Patient B will be sent a letter offering credit protection services due to full name and full SSN being exposed.NOTE: There were a total of 24 Mis-Mailed incidents this reporting period. Because of repetition, the other 23 are not included in this report, but are included in the "Mis-Mailed Incidents" count at the end of this report. In all incidents, Veterans will receive a notification letter and/or credit monitoring will be offered if appropriate.
Outcome:
Since the Privacy Officer was unable to determine who the responsible party was, all employees at the CBOC were educated on safeguarding information.
