Release of Information (ROI) staff was requested to process a release of medical information to a VA employee/patient, per a valid authorization. The ROI staff left the report, unprotected, on employee/patients desk where it was visible to anyone in the area. The report contained the employee/patient's name, address, full SSN and protected health information (PHI) Update: 12/09/11:The employee/patient will receive a letter offering credit protection services.
Outcome:
Employee retook training and was counseled by Privacy and supervisor regarding appropriate protection of sensitive information.
