Issue:
A VA employee called 911 to report a threat to a staff member from a Veteran. She disclosed the Veteran's name and full date of birth. The Veteran was in an adjoining mall and not in the clinic at time the call was made. Update: 04/29/13:The Veteran's medical record was accessed without a need to know. The Veteran did not enter the clinic, but was at the mall going to a store. Police were called and the Veteran was questioned at one of the stores. The Veteran will be sent a HIPAA notification letter.05/22/13:This was determined to be non-HITECH reportable by VHA Privacy Office.
Outcome:
Employee has been educated and training will be provided to entire clinic on 5/20/13.
