Issue:

Today, 03/13/15, the Privacy Officer (PO) received a report from Employee A who had received Employee Bs sensitive documents by mistake. Employee A had stopped into Human Resources (HR) on 03/12/15 to pick up copies of insurance enrollment information. Employee A was handed an envelope with their copies as well as the life insurance enrollment form of Employee B. Employee A saw the employee Bs full name and full SSN. It was handed back to the HR rep at the desk and informed of error. The same situation had occurred to Employee A in New Employee Orientation two weeks ago with receipt of another employee's documents. They were similarly returned to the same HR rep. Additional notification and investigation, to ensue.

Outcome:

03/19/15: The Incident Resolution Service Team has determined that there was a privacy violation, but since the data never left VA control, this is not a data breach.
