Issue:

The VA Health Resource Center (HRC) notified the facility Privacy Officer they received a telephone call from a citizen who reported that they received a billing statement through the mail belonging to a Veteran. The HRC instructed the citizen to return the statement to their office. Once received the HRC determined the statement was mailed on behalf of the Biloxi VAMC by the vendor under contract to the VA Corporate Data Center Operations (CDCO) in Austin, Texas. The Vendor data is based on the information contained in the facility's VistA system.VA Health Resource Center appropriately destroyed original documents.Facility action - update Veteran's address to "address unknown" until a correct address is obtained from Veteran. Update: 03/06/12:The Veteran will be sent a HIPAA notification, because his name and medications were disclosed.

Outcome:

Documentation retrieved and destroyed by shredding. Veteran's address updated/corrected in Vista.
