Issue:
VA Clerical Staff printed lab orders and handed to a Veteran. The lab order printed however was for the wrong Patient. The lab orders were given to Quest diagnostics. The error was discovered by the VA lab manager upon looking for results on the correct Patient. The original lab order was recovered. The Veteran who rec'd the incorrect documents was unaware that an error was made. Update: 06/20/11:Patient A will be sent a letter offering credit protection services.
Outcome:
Employee who made the error was re-educated on privacy issues.
