Issue:

On, 06/04/14, the Pharmacy Manager reported a privacy incident to the Privacy Officer (PO). Veteran B stopped in the Pharmacy to inquire why his medication dosing had changed. Review of the bottle by supervisor shows that despite same controlled medication the prescription name was for another Veteran. The dose was half the usual dose which the Pharmacy re-processed. Veteran A was subsequently contacted. He had received and noticed Veteran Bs identity on medication by mail. He said the medications were taken to Albertsons Pharmacy to be destroyed. Requested that his medications be refilled; referred to physician for review.

Outcome:

06/05/14: The Incident Resolution Team determined that bothe Veterans will receive a HIPAA letter of notification since their protected health information (PHI) was compromised.
