Issue:

A Veteran complained that about a year ago, he went to the VA clinic and had some blood work done. Shortly after that he received a phone call from his grandparents concerned about his blood work results. The Veteran states that his mother, who is a VA clinic employee, informed his grandparents about his medical results. The Privacy Officer (PO) will further investigate. Update: 11/19/12:Based on initial phone call to the PO on 11/15/12, the Information Security Officer (ISO) marked the record sensitive for tracking future access.11/29/12:The clinic investigation is completed and the employee involved (a lab employee) admitted to talking with the Veterans grandfather (her father) about the Veterans laboratory results. The employee denies ever going into the Veterans medical record. The employee has completed retraining on Privacy and Information Security. The Clinic Medical Officer has recommended ten days suspension. The veteran will receive a HIPAA letter of notification.

Outcome:

The employee has completed retraining on Privacy and Information Security. The Clinic Medical Officer has recommended ten days suspension. HIPAA Notification Letter mailed to affected Veteran.
