The Regional Office contacted me about a document that was in the Claim folder. The Privacy Officer stated that the document in Patient A's record was his, but on the backside of the page was full name and last four of SSN stating he had no-showed his appointment Update: 09/24/12:The backside of the page contained the name and last 4 of SSN of Veteran B. Veteran B will be sent a letter of notification.
Outcome:
Information was retrieved. Staff were educated. Notification letter was mailed.
