Issue:
Palo Alto Privacy Officer reported a FedEx package sent from a VA Pittsburgh employee containing two Veteran consent forms was never received. This PO is attempting to contact the VA Pittsburgh employee to obtain further details. Update: 05/18/12:The tracking number has been received and tracked via FedEx. The tracking indicates the the shipment left this facility on 1/30/2012 and was last accounted for upon departure from a FedEx location on 2/1/2012. At this time, it has been determined the package is lost. lost by the FedeX05/30/12:even though not fault of VA two Veterans will recieve a offer for credit protection services.
Outcome:
The employee has been re-educated about reporting incidents immediately to the Privacy Office and/or Information Security Office.
