Issue:

On 10/31/12, the Privacy Officer (PO), Nashville Campus, was notified of the following: On 10/30/12, while writing a courtesy violation in the parking garage (Nashville Campus), the VA Police Officer observed a computer print-out with patient information, located in the trunk of the vehicle, in clear view of individuals passing by. The print-out contained a Veteran\xe2\x80\x99s full name, full social security number, and clinic appointment date/time. The vehicle is registered to the Physician Assistant (PA), Dermatology Clinic (Nashville Campus). The VA Police Officer interviewed the PA, Dermatology Clinic (Nashville Campus) regarding the incident and was informed: \xe2\x80\xa2\n\xe2\x80\xa2\n\xe2\x80\xa2 While in clinic, PA printed her notes on this Veteran and placed the document in her lab coat. PA inadvertently failed to remove the document before placing her lab coat in the trunk The document fell out of the lab coat and into the trunk VA Police Officer completed a Uniformed Offense Report (UOR) #2012-10-30-1101-5129 and reported the incident to Chief, Medical Service and Privacy Officer (PO). VA Police Officer confiscated the document and gave it to the PO. Update: 10/31/12: The Veteran will receive a letter offering credit protection services.

Outcome:

11/13/12 - Provide has been re-educated on the importance of safeguarding patient information. CM letter mailed.
