Issue:

A wristband was placed on the wrong patients in the Emergency Department (ED). Veteran A was seen in the Emergency Department, Veteran states a wrist band was placed. She went home and realized the wrist band she had on was not her information or her photo ID. The information on the armband was of Veteran B. Veteran C was seen in the ED on and returned home with the wrist band and found it had another Veterans information on it. The Veteran identified on the wrist band: Veteran D. Update: 03/15/13:Veterans B and D will be sent offers for credit protection services.

Outcome:

Supervisor provided documentation that Staff have been re-educated on properly identifying patients prior to placing wristbands on patients.
