Issue:

VA Social Worker reported that she is unable to locate her files from a Nursing home visit on 11/20/14. Social Worker brought a client list with the full name, DOB and full SSN of the patients along with notes from CPRS to her visit. Social worker stated that the last time she recalls seeing the list was Thursday 11/20/14 around 6:00 PM when she completed her visit at the Nursing home and realized on 11/21/14 evening that the list was not where she had placed it in her bag. Social Worker stated that she contacted the Nursing facility but has not heard any information if the list was located and has searched all of her files in her bag and is unable to locate the list.

Outcome:

11/25/14: The Incident Resolution Service Team has determined that 13 Veterans will be sent letters offering credit protection services. 01/12/15: On further review only 12 Veterans information was lost.
