Issue:
A Veteran returned two separate lab results (for different Veterans) to a Health Administration Services (HAS) clerk indicating that had been mailed to his residence and sent in the same envelope as his own lab work. The HAS clerk failed to get the Veteran's name and any other details surrounding the circumstances of their arrival at the Veteran's residence. One letter was lab results, the other a two-page CT scan result. The letters were dated 03/07/12 and 03/08/12 and were returned to the HAS clerk on 03/12/12. The Primary Care Supervisor generated a Report of Contact and sent to Privacy Officer through interoffice mail. Update: 03/14/12:Two (2) Veterans will be sent a notification letter.
Outcome:
Emphasizing to all employees the need to double-check envelope contents with addressee before mailing. Notification Letters sent to affected Veterans.
