Issue:

Today, 12/29/14, the Privacy Officer (PO) was informed by Veteran C in the Patient Advocate Office that his son, Veteran A, received medical records for Veteran B. When Veteran A was hospitalized in October 2014, he requested his medical records with assistance from father, Veteran C. In late November, medical records of pages 29-118 were mailed to him of Veteran B. None of Veteran As records have been received. Veteran C returned the records today with the mailer and original VAF 10-5345. Recovery Actions: The Health Information Management Service (HIMS) Chief initiated STAT records processing and oversight to provide Veteran As medical records. Additional notification and investigation to ensue.

Outcome:

12/29/14: The Incident Resolution Service Team has determined that Veterans A and B will be sent a letter offering credit protection services.
