Issue:
Veteran A received letter from Release Point in regards to his medical records from private provider. When he called them back they stated they were gathering medical records for his insurance policy. They stated they already had his VA records. he did not authorize the records. Confirmed in DDS/ROI software that no request was received from Release Point. Contacting Release Point for more information Update: 10/16/12:Veteran A will be sent a notification letter due to PHI being release without authorization.
Outcome:
Unable to determine who released records if VHA or VBA. Continue to research.
