Issue:
VA employee discovered spreadsheet with 15 Veteran/patient last names and last 4 SSN unattended in Veteran/patient's room. Update: 01/06/12:It is not known how long the spreadsheet was in the room, however, based on the patients listed it appears to have been left in November of 2011. It is not known how the information got there. It was found in a drawer of a bedside table by a nursing assistant.
Outcome:
VA employee that left information unattended was not identified. Staff members in the vicinity of the incident were notified/educated.
