Issue:

Veteran A was given his medical information to bring to an appointment at the referral DoD facility. When he gave the information to his provider of care, the provider noticed there was other Veteran patients' information on the backside of his copies. There are 7 Veterans affected by this mishandling/disclosure. The information disclosed included the 7 Veterans' name, date of birth, full SSN and protected health information (PHI) Update: 05/09/11:The information included the Veterans names, SSNs, date of birth and protected health information., The 7 Veterans will receive letter offering credit protection services.

Outcome:

The employee has been counseled on the incident and punitive action taken. Additional education for the CBOC staff was done and the affected employee will be required to take privacy and security training again. While the incident had extenuating circumstances with the supply of paper, it was still inappropriate and unacceptable. Strictly human error that could have been avoided. The staff have been counseled on the practice of re-using paper even without PHI on it.
