Issue:

Patient A came in for a visit to his yellow team and new medication was given to him. He picked up the medication. When he got home his wife went to set up the medication and noticed he had been given the wrong medication. He was given medication Patient B who was waiting in the lobby waiting for his medication. The wife called the medical center and brought the medication back. Update: 05/23/12:Due to date of birth being exposed, Patient B will be sent a letter offering credit protection services.

Outcome:

Pharmacy staff notified that they should be more aware of names on RX packages.
