Issue:

The Privacy Officer (PO) was notified of this incident by a Fee Basis employee: Veteran A telephonically contacted the VA Fee Basis Section to report that when he received a letter in the mail from the Fee Basis Department and when he opened the letter, he realized it contained the Fee Basis Authorization information of Veteran B. Veteran A agreed to mail the mis-mailed documents to this Privacy Officer. The PO cannot state for sure what specific protected health information (PII)/protected health information (PHI) of Veteran B was disclosed until those documents are received back; however, the documents did at the minimum contain Veteran B's full name, last four of his SSN and home address. PSETS will be updated as soon as it is learned if any other PII was involved.

Outcome:

08/19/14: The Incident Resolution Service Team has determined that Veteran B will be sent a HIPAA notification letter due to Protected Health Information (PHI) being disclosed.
