Issue:
Veteran A called to say that he received Veteran B's lab results in the mail. Both Veterans share the same exact name, but different SSN's. Update: 06/25/12:Veteran B will be sent a letter offering credit protection services, as his full SSN, lab results, and other PHI were disclosed.)7/03/12:An appeal was filed. The DBCT reviewed and granted the appeal based on the evidence submitted which stated that the full SSN was not disclosed. The Veteran's name, address and lab results were disclosed. Credit protection services are not required but Veteran B will receive a letter of notification.
Outcome:
All employees are being advised to carefully crosscheck last four of SSN when making VISTA entries.
