Based on interview and record review, the hospital failed to ensure that Patient 1's protected health information (PHI), was kept confidential from anyone not authorized access.Findings:An interview with the Director of Risk Management (DRM) was conducted on 11/26/12 at 2:15 P.M. The DRM stated that a pharmacy technician inadvertently gave Patient 1's medication and prescription paper to Patient 2. The prescription paper indicated Patient 1's name, date of birth, home address, phone number, and medication.A review of the hospital's policy and procedure titled Confidentiality of Information indicated that, "Sensitive information collected and/or generated within (hospital name) shall be maintained in such manner that access to it is restricted to those with a need to know."
