Issue:

Patient A received a prescription package intended for Patient B. Patient Bs name and type of medication was compromised. Patient A reported the incident to the medical center and a replacement has been requested for Patient B. The Leavenworth Consolidated Mail Outpatient Pharmacy (CMOP) investigation determined that UPS software system error occurred during the UPS manifesting segment at the CMOP which resulted in the printing and application of a UPS label with an incorrect address. The UPS database was purged and UPS is investigating the cause. CMOP will treat this as a CMOP packing error. Update: 05/01/12:Patient B will be sent a notification letter due to PHI being disclosed.
