Issue:

Based on interview and record review, the facility failed to ensure a patient's (Patient A) lab order was given to the intended patient. That resulted in Patient B receiving and having access to Patient A's PHI (Personal Health Information).Findings:On March 7, 2013, a visit was made to the facility for the purpose of investigating a self reported breach. On March 7, 2013, at 12 p.m., a review of Patient A's record was conducted. On February 8, 2013, at the facility's Women's Center, the physician gave Patient B Patient A's one-page lab order sheet that contained Patient A's name, date of birth, address, phone number, insurance information, and medicial record number. The breach was discovered when Patient B took the lab order sheet to the laboratory.
